	I have read the Lakes of Bent Tree animal policy and understand that bringing an animal on the property without prior written approval from the board of directors is subject to an automatic $2,000 initial fine plus $200 per day (click here to see Pet Policy, Dallas County Deed Records).

	My pet has not been involved in a dog-to-human or dog-to-dog injury incident. I will report any such incidents to the board of directors.
